The FAI Index (Free Androgen Index) is a calculated marker used to estimate the amount of biologically active – free – testosterone in the body. It is derived by dividing the total testosterone level by the SHBG (Sex Hormone Binding Globulin) level and multiplying the result by 100. FAI is commonly used in both men and women to assess androgen balance, particularly in cases of suspected hypogonadism or conditions such as polycystic ovary syndrome (PCOS).

The advantage of FAI is its accessibility, especially in laboratories where direct measurement of free testosterone is not available.
