The reverse triiodothyronine (rT3) blood test measures the level of an inactive form of the T3 hormone, which is produced from thyroxine (T4) mainly in peripheral tissues. Unlike active T3, rT3 has no significant hormonal effect, but its concentration may reflect changes in thyroid hormone metabolism.
rT3 levels may increase, for example, during severe acute or chronic illness, prolonged stress, fasting, significant calorie restriction, or certain metabolic conditions. The test is therefore mainly used as an additional marker in the comprehensive assessment of thyroid function and thyroid hormone metabolism.
rT3 measurement may be particularly useful in situations where TSH, fT4, and fT3 results are difficult to interpret or do not correspond with the clinical condition. The result should always be evaluated together with other thyroid parameters, especially TSH, fT4, and fT3.
